Job Description – Team Leader Family Centred Primary Health Care Outreach Team
THIS POSITION IS BASED AT WALGETT ABORIGINAL MEDICAL SERVICE
ACCOUNTABILITY
Responsible to the WAMS Board of Directors via the Chief Executive Officer
JOB SUMMARY
- Work with FCPHC Aboriginal Health Workers to provide an outreach Family Centred Primary Health Care to Aboriginal people in Goodooga, Pilliga, Gingie and Namoi.
- Co-ordinate the work the of the Medical and Allied Health Care Teams to assist in the provision of the Family Centred Primary Health Care to those outlying areas covered by this program.
- Lead a multi-disciplinary team in the provision of high quality Family Centred Primary Health Care to outlying communities.
QUALIFICATIONS AND EXPERIENCE
ESSENTIAL:
- Registered Nurse List A
- Current NSW Driver's Licence
- Experience providing Primary Health Care across a range of settings
- Previous experience in;
- working with Aboriginal communities
- program planning
- good written and oral communication skills
- Demonstrated ability to lead a Multi-disciplinary Primary Health Care Outreach Team
DESIRABLE:
- Community health/education/mental health qualifications
- Report and submission writing skills
· Computer literacy with experience in Works for Windows
- Midwifery and/or Early Childhood Qualification
DUTIES:
- Maintain confidentiality and privacy in all matters relating to WAMS at all times
- Work with members of the Family Centred Primary Health Care Team to
- assess community needs, resources available and set priorities
- plan, implement and evaluate programs which are designed to meet the needs of the community
- Contribute to the WAMS continuous quality improvement process and ensure that the Family Centred Primary Health Care service works within this framework at all times
- Maintain statistics in accordance with WAMS performance indicators
- Represent WAMS and the local community at the national, state regional and local level
- Liaise with the other staff of WAMS to ensure that;
- the safety of clients, staff and other users of WAMS is maintained
- resources are used effectively and efficiently
- adequate supplies are maintained
- Assist the Family Centred Primary Health Care General Practitioner and Allied Health Care staff to ensure a high quality of client care is provided to the communities serviced by this team.
- Work with Medical and Allied Health staff to ensure all Medicare entitlements are claimed by WAMS.
- Ensure that professional and personal development occurs whilst working with WAMS
- Liaise with all members of the community in the towns serviced by WAMS Family Centred Primary Health Care Team to ensure that each community receives a high quality of care
- Maintain accurate and current documentation of matters concerning clients and WAMS
- Work with the Family Centred Primary Health Care staff WAMS to ensure that;
- the safety of clients, staff and other users of WAMS
is maintained
- adequate supplies are maintained
- resources are used effectively and efficiently.
